Objectives:
To promote Wellness with education and self motivation / participation using traditional and complementary health care modalities.
Clinics:
- Cardiovascular Disease and Cholesterol Clinics with Nancy Cassella FNP-C
- Emphesema, Asthma, and Respiratory Care with Erika Snowman PA-C
- Diabetes and Nutrition with Lisa Starkweather FNP-C
- Nutrition and Weight Management (NWM)
- Chronic Disease self management (DM, CVD, Asthma / COPD)
- Tobacco product use cessation
- Physical Therapy & Rehab – Carol Lane : Monday, Wednesday, and Friday
- Prenatal Health
- Young Mothers and New Born
- Young Peoples Health (Age Groups: 1-5, 6-9, 10-14, and 15-19)
- Senior Health (Age Groups 65-75, 75-85, 85Plus.)
Schedule:
Mondays:
8:15 a.m. - 9:00 a.m. (DM-Adults)
3:45 p.m. - 4:15 p.m. (NWM)
Tuesdays:
8:15 a.m. - 10:15 a.m. (P.T & R)
Wednesdays (1 & 3):
10 a.m. - 11:30 a.m. - Pain Mx-consults only – 30 minutes each
1:30 - 4:30 – Office visits – Pain Patients only.
Thursdays (1, 3 & 5):
8:15 a.m. - 12:15 p.m. (Podiatry)
Fridays:
8:15 - 9:00 hrs (DM – Adults)
3:45 p.m. - 4:15 p.m. (NWM Pediatric/young/Teens)
Procedure clinic schedule:
(M, T, F 12:30 p.m. - 2:00 p.m. and Friday 10:30 a.m. -11:30 a.m.)
MA/Provide Protocol:
- Patients will be scheduled as per clinic hours.
- MA will give any printed information.
- Provider will have interactive session with patient including demos (30 minutes).
- MA will give any individual information to each patient, answer any questions, schedule next appointment.
- MA will reenter a patient in the same program or a different program based on the goal achieved or to accomplish a new set of goals.
- Graduation from the health program with certificate.
Diabetes Clinic Protocol:
- MA will check weight/BMI/BMI Index.
- Enter recent result on CO-OP card.
- Patient target result vs. current results.
- Set new realistic goals
Topics include:
- MA will measure height/weight/BMI/BMI index
- Target weight vs. current weight
- New realistic goals
Topics to be covered:
- What is ideal weight and why it is important
- Obesity as health issue and important factor for diseases
- Food types – Carbs, fats, proteins, etc.
- Counting calories
- Importance of exercise
- When, where, and how to exercise
- . Diets – recommended and not recommended
- Demonstration of exercises and equipment
Pain Management – First & Follow Up Visit Protocols:
- 1st Wed of every month – 10-11:30am, new patients and pain consults (30 min)
1:30-4:30pm, follow-up visits (15 min)
3rd Wed of every month - 10-11:30 & 1:30-4:30pm – follow-up visits (15 min)
Avoid booking any routine labs work from 1:30-4:30 on these Wednesdays.
First Visit:
Patient arrives 15-20 minutes prior to appointment time.
Receptionist will give patient intake form and medication agreement.
MA will check patient in with weight and vital signs in a room with proper exam table. The patient is advised to undress except underwear.
Provider may order random drug screen to validate patient's history of medication use.
Follow-up Visits:
- MA will check patient in with weight only.
- All pain medications will be checked and recorded by MA
- Random drug screen will be done based on that day criteria by the provider.
- All necessary pain medication prescriptions (one month at a time) will be given to the patient by provider and checked by patient before leaving the facility.
- Patients must come on Wednesdays unless there is a valid reason.
