AFFORDABLE PRIMARY CARE FOR PATIENTS WITH HIGH DEDUCTABLE/CATASTROPHIC COVERAGE

The enclosed plan for coverage is offered for patients of Dexter Family Practice as a “cash basis” health maintenance plan. This coverage applies to all patients with very high deductibles or catastrophic coverage policies.

A contract will be signed on or around your birthday for the coverage chosen with annual renewal. Contracts will be prepaid with no refunds. There are a variety of plans to choose from with different levels of coverage. Your healthcare provider will be more than happy to sit with you to help chose the plan which is right for you.

PLAN DESCRIPTION BASIC SILVER GOLD PLATINUM
Annual Physical with choice of 2 tests from list* $165.00**      
Basic plus 2 office visits and 3 tests from list*   $325.00**    
Silver plus 4 office visits and 4 tests from list**     $465.00**  
Basic plus any of the following: Colonoscopy with biopsy, EGD with biopsy. Any hospital charges will be separate and the patients responsibility.***       $990.00**

* Prostate Screening/Thyroid Screening/Cholesterol Screening/Liver Function/Basic Metabolic Panel
** All Packages payable in advance and non-refundable
*** Pathology charges for PAP tests may be paid in advance for additional savings
