Dexter Family Practice is pleased to announce being selected as part of an elite group of primary care practices across the state to participate in a nationwide demonstration project, Patient Centered Medical Home. The Medical Home concept initiated by the federal government is a 2 year study on practice transformation toward the Patient Centered Medical Home. Along with the other 22 practices selected, Dexter Family Practice will continue to strive toward quality primary care centered healthcare using the following features:

  • Open Access/Walk-in Care              

 

  • Team Approach to Care       

 

  • Self-Assessment for Our Patients

                                                            

  • Patient Portals for Access to Scheduling and Test Results     

 

  •  Patient Participation through our "Patient Advisory Board" which consists of patient members of the leadership team who participate in staff meetings and improvement implementation.
