Dexter Family Practice is pleased to announce being selected as part of an elite group of primary care practices across the state to participate in a nationwide demonstration project, Patient Centered Medical Home. The Medical Home concept initiated by the federal government was a 2 year study on practice transformation toward the Patient Centered Medical Home. Dexter Family Practice was one of 26 practices selected to be in the pilot program and Dexter Family Practice has achieved Level III status by delivering quality primary care centered healthcare using the following features:
